1. Request that the following names person he apprehended and interned in prison for trial as a Suspected War Criminal:

b. Rank of Title: Captain, IJA
c. Units to which attached: Western Army Headquarters, Fukuoka City
d. Dates and Places of service: 1942 to surrender
e. Hme address (if known): Kokura-shi, Imonoshi-machi, 103 Benchi
f. Synopsis of Charges presently prepared, subject to amendment, against subject Suspected War Criminal: As Adjutant Section Officer with duty of caring for American Prisoners held, he knowingly released them to Kyushu Imperial University for experimental vivisection, gave two prisoners anesthetic before they left.
g. Synopsis of Evidence to support charges: Confession and statements of co-perpetrators
h. Anticipated place of trial: Yokohama
